The Long Live Idaho campaign is distributing health and wellness items throughout Fall of 2022. Each item has an attached postcard with a QR code that leads to a quick survey to tell us how you plan to use your item and an opportunity to opt-in to a follow-up survey and a chance to win an $85 value wellness basket!

You must complete the original survey and opt-in to the follow-up survey to be entered to win a wellness basket.
